Record a Therapeutic Observation

Steps to open and complete a therapeutic observation form in Access Rio Observations, including the Quick Observation option.

Therapeutic observation tasks are started in the same way as other observations in the app. This article walks you through how to open a therapeutic observation form, complete it, and save the record.

Open a therapeutic observation task

You can open a therapeutic observation task from:

  • The Tasks List screen.

  • The To Do list on the Patient Profile screen.

  • The Add Record button.

Tap the task card you wish to record an observation for and confirm the patient's identity. The form will be shown as configured by your Trust.


Complete and save the observation

Quick Observation

On forms that have been configured to support it, a Quick Observation button is shown at the bottom of the screen. Tapping Quick Observation allows you to save the form with only key fields completed. This can be useful where a rapid check is required.

If completing a full observation, or the Quick Observation button is not present on your form, all mandatory fields must be completed before the record can be saved.


Save the record

Once all required fields have been completed:

  1. Tap Review Observation. The Observation Summary screen is displayed, providing an overview of the recorded values.

  1. To edit the observation, tap the back button. To save, tap Save Observation.

A confirmation banner is displayed once the record has been saved to the device. The record will appear in the patient's History timeline.

πŸ“ŒNote: The Observation Summary screen may not be available for all therapeutic observation forms. Where it is not available, the record will be saved directly on confirmation.
