Ways to add a new observation
You can start recording a new observation in three different ways:
From the Patient Profile Task View.
Open the Patient Profile.
Click on the required task card to begin recording the associated observation.
Using the Add Record button.
Click Add Record.
Select the required observation type. This allows clinicians to record ad hoc observations, meaning observations not linked to a task.
From the patient task list.
When viewing the patient task list either on first access or when opening the profile, click any task card to begin recording.
Identity confirmation check
For patients without a profile photo, the system will display an identity confirmation prompt before allowing access.
Confirm that you are accessing the correct patient's information.
Once confirmed, this preference is remembered until you log out.
Physical Observation Preview
When starting a new physical observation, a preview screen is displayed showing:
The latest Early Warning Score (EWS) chart, based on the patient’s Rio EPR admission record.
You can:
Tap the chart to open a full screen view for more detailed data.
The patient banner remains visible to ensure patient context is maintained.
To proceed, tap Perform Physical Observation.
Dynamic Early Warning Score Indicators
Each field displays a colour coded number representing the early warning score for the entered value.
These scores update dynamically as you complete the form.
Mandatory Field Validation
If required fields are missing:
A validation pop up appears when you tap Review Observation.
This lists all fields that need to be completed before continuing.
Partial Observations
If you cannot take all required measurements:
Select the Partial Observation checkbox.
This records the observation as a partial entry.
Monitoring Frequency
After completing the observation values, choose a Monitoring Frequency before reviewing.
Reviewing and Saving the Observation
The Observation Summary screen appears when you tap Review Observation.
📌Note: This screen is only available for physical observations.
From here you can:
Click back to amend the record, or
Tap save observation to complete and store the entry.
