Before you begin
You must have access to the Observation Management.
You must have the appropriate permissions to edit or delete templates.
View form templates
From the main navigation menu, select Forms.
The list displays all available templates, including who created each one.
If a description is shortened with an ellipsis (...), hover over it to view the full text.
Use the search bar to find a template by name.
Select any column header to sort the data.
Sorting applies to the full dataset, not just the current page.
Preview a form template
Locate the template in the list.
In the Actions column, select the preview icon.
Review the layout and questions as they will appear to clinical staff.
📌Note: The preview is a live render of the form. This means it reflects exactly how the form behaves in the mobile application.
Edit a form template
Find the required template in the list.
In the Actions column, select the three dot menu.
Select Edit.
Make your changes and save the template.
View FHIR JSON structure
Locate the template.
Select the three-dot menu in the Actions column.
Select Preview FHIR JSON to view the underlying structure.
Delete a form template
Find the template you want to remove.
Select the three dot menu in the Actions column.
Select Delete.
Confirm the deletion.
⚠️Important: Deleting a template does not permanently remove it. The template is moved to the Deleted templates tab
Restore a deleted template
Go to the Deleted templates tab.
Locate the template you want to restore.
Select the three dot menu.
Select Restore.
The template will be returned to the active template list.
