Creating a Form Template

Create and configure a new form template so it can be used in the mobile application.

Before you start

  • You need access to the Forms Management page.

  • Make sure you understand what fields and structure your form requires before creating it.


Create a new form template

  1. Go to the Forms Management page.

  2. Click Create Form.

  3. Enter a Title and Description for the template.

    • Both fields have character limits enforced by the system.

  4. Set the Status as required.

  5. Click Next to move to field configuration.


Add and configure fields

  1. Choose a field type from the available list, up to 15 field types are supported.

    • This includes options such as numeric fields, choice fields, and therapeutic OBS picker fields.

  2. Configure the field as needed:

    • For number fields, set minimum and maximum values.

    • For choice fields, define the available options.

  3. Repeat to add all required fields.

Manage fields

  • Drag and drop fields to reorder them.

  • Use the copy option to duplicate a field with the same settings.


Add conditional logic

  1. Select Conditional logic.

  2. Click Create rule.

  3. Configure the rule:

    • Select the condition field.

    • Define the value it must meet.

    • Choose which field to show or hide based on that condition.

  4. Add additional rules if required.

πŸ“Œ Note: These rules run in the background within the mobile application and are not visible as form fields to users.


Preview and publish the form

  1. Click Next to open the preview.

  2. Review the form carefully to make sure:

    • All fields display correctly.

    • Conditional logic behaves as expected.

  3. Click Complete to finalise and publish the template.


Result

The form template is now published and available for use in the mobile application.
